Child Malnutrition in Chad
Chad, a country located in Central Africa, faces one of the highest levels of child malnutrition worldwide. A meta-analysis of child malnutrition in sub-Saharan Africa from 2006 to 2016 found that 39.9% of children in Chad suffered from stunting and 28.8% were underweight. Extreme weather events and conflict in the country exacerbate food insecurity, making it more difficult for many families to provide adequate, nutritious diets for their children. To help improve children’s health and reduce food insecurity, four recent initiatives are tackling child malnutrition in Chad.

Scaling Up Nutrition

Scaling Up Nutrition (SUN) is an organization that collaborates with low- and middle-income countries’ governments to organize malnutrition prevention efforts. In 2017, SUN developed partnerships with five civil society organizations in Chad focused on improving nutrition. SUN has also established six local Civil Society Alliance offices across different provinces of the country. With SUN’s support, these organizations adopted nutrition as an integral part of their development plans. SUN has also trained and mobilized 35 radio presenters and journalists for nutrition communication, who continue to help raise awareness on malnutrition across the country.

Collaboration with UNICEF and the UK

Through its Department for International Development, the U.K. committed £4 million to a collaboration with UNICEF to reduce acute malnutrition in Chad in 2018 and 2019. Using this grant, UNICEF provided therapeutic milk, Ready-to-Use Therapeutic Food and essential drugs to 58,670 children across 20 provinces nationwide.

UNICEF also used the DFID grant to develop more sanitary and hygienic health centers, improving 30 facilities across the country. This development benefited an estimated 40,000 mothers and caregivers of children suffering from acute malnutrition.

Zafaye West Health Center

A nutrition project that UNICEF and the U.K.’s Foreign, Commonwealth and Development Office sponsored supports the Zafaye West Health Center. The project selected N’Djamena, where the health center is located, as a priority province in Chad for nutrition aid because a 2019 survey detected a high prevalence of acute malnutrition in the area.

Community volunteers from the center travel door-to-door to reach out to mothers, encouraging them to visit the health center to check up on their children’s health and engage in educational campaigns. The campaigns educate mothers on the importance of balanced diets for their children and teach them nutrient-dense, affordable recipes to prepare. The nutrition project has saved 43,000 children, located within the six target provinces it serves, from acute malnutrition as of June 2021.

The World Food Programme (WFP)

The World Food Programme is an organization that provides food assistance across more than 80 countries worldwide. WFP helps provide nutritious meals to 120,000 school children in the Sahel, the region of Africa where Chad is located. The organization also feeds 15,000 children in the Lake Chad region through an emergency school meal program.

In addition, WFP helps prevent child malnutrition in Chad among 6-month-olds to 2-year-olds by providing cash-based nutrition support to their families. This support provides families with more stable access to nutrient-dense foods.

Although many children in Chad currently face malnutrition, these four initiatives are making progress in eradicating this issue. With this support, child malnutrition in Chad may decline in the years to come.

Stunted Growth in Children
In 2019, 149 million children under the age of 5 around the world experienced stunted growth. Children that stunted growth affects are 33% less likely to evade generational poverty as adults. By continent, 36% of children in Africa under the age of 5 are malnourished. Around 40-45% of “all preventable child deaths” are due to undernutrition. In 2012, this meant that more than 6 million children died from stunted growth disorders.

Stunted Growth in Children

Malnutrition in the early stages of a child’s life causes stunted growth. Stunting correlates with impaired physical growth and cognitive development, a weakened immune system, higher mortality rates and overall poor health. Stunted growth is a chronic condition that appears within the first two years of a child’s life.

Children who experience stunting are more likely to be fatigued and less curious, which naturally lessens their psychosocial development. Additionally, they tend to face disciplinary challenges as well as possess less developed motor function and social skills. These challenges perpetuate the cycle of poverty, as stunted growth in children leads to higher dropout rates and a 22% reduced earning capacity in the workforce.

While the effects of stunted growth are largely irreversible, reducing malnutrition will lessen underdevelopment and other illnesses that stem from malnourishment. The World Health Organization (WHO) has plans in place to reduce the prevalence of this disease by 40% by the year 2025.

Physiological Explanation

Malnutrition causes diminished cognitive function and psychosocial adversity in children by altering neurological function. This, in turn, leads to reduced income as adults. Dendrites are neurons that communicate with nerve cells and pass on signals in the brain. Malnutrition in young children decreases the density of dendrites in the brain and therefore reduces the number of neurons. This process negatively affects critical brain development such as memory formation, locomotor skills and other neurological functions, which are critical to healthy brain development.

Links to GDP Growth

According to the Global Panel on Agriculture and Food Systems for Nutrition, malnutrition drains the global economy of approximately $3.5 trillion per year from lost productivity. Individuals often experience a lack of brain development during the first years of their lives from undernourishment. They later suffer from diminished productive capacities in their livelihoods. The Global Panel reports that a 3% to 16% annual GDP loss results from malnutrition. Simply put, better-nourished children grow into more productive adults.

Policy Changes and Solutions

As with many public health problems worldwide, foreign aid investments may be a critical starting point for reducing malnutrition and stunted growth in children in poor regions. The Food and Agriculture Organization of the United Nations (FAO) estimates that if the U.S. invested $1.2 billion per year in the global fight against malnutrition, the decrease in deaths and an increase in “future earnings” (GDP income and relative economic benefits) would generate $15.3 billion for the U.S. per year. This calculation represents a thirteen to one benefit-to-cost analysis.

An Ethical Approach

Much more than an economic incentive, there is a moral imperative to improve nutrition globally. Eliminating malnutrition would increase the overall health of populations. Poor communities that lack consistent access to nutritious food and healthcare would particularly feel this effect.

Research shows that, while the impaired cognitive state is not necessarily permanent and can improve incrementally, there typically remains overall “cognitive dysfunction” in stunted children in comparison to healthy children. The FAO’s recommendations include dietary supplements, food fortification (the addition of nutrients to food to increase the nutrient content) and biofortification (agricultural practices that incorporate DNA recombination to augment nutritional content in primary crops).

Along with FAO dietary solutions, the WHO has developed policy aims to reduce stunted growth in children by 2025. Its policies include collaboration between organizations such as Scaling Up Nutrition (SUN), which works to reduce global malnutrition and the health disorders that accompany malnourished children. SUN helps countries develop and implement government policies to improve nutrition during the critical period before a child’s second birthday. Through collective action efforts, SUN, WHO, governmental entities, the U.N. and individual stakeholders are joining forces to eliminate malnutrition.

SUN Movement
Malnutrition accounts for nearly half of all deaths among children under five. While the majority of these deaths occur in Africa and Asia, the loss of human life due to hunger and malnutrition is a global burden. Malnourished children are more likely to get sick, suffer from abnormally severe symptoms of common illnesses and die from otherwise preventable illnesses. Thankfully, organizations such as the SUN Movement work to reduce this hunger-related child mortality rate.

Malnutrition and Infection

The link between malnutrition and infection can create a cycle wherein poorly nourished children have a weaker immune system, which in turn deteriorates their nutritional status. Malnutrition can also stunt a child’s growth, predisposing them to cognitive disabilities.

Hunger and malnutrition take a particularly severe toll on the developing world, where one out of six children (about 100 million) are underweight, one in three children are stunted and 66 million children go to school hungry.

The SUN Movement

Scaling Up Nutrition, or SUN Movement, is a worldwide campaign to alleviate hunger and malnutrition. SUN aims to unite governments, the United Nations, civil society, researchers, donors and business into a cohesive movement to improve global nutrition.

Focusing on the goals established at the 2012 World Health Assembly, SUN movement identifies four strategic processes as the major institutional changes needed for scaling up nutrition worldwide:

  1. Endorsement of National Nutrition Policies that Incorporate Best Practices. Newly enacted laws and policies should reflect proven interventions while paying special attention to women and their role in society.
  2. Sustained Political Commitment and Establishment of Functioning Multi-stakeholder Platforms. Improving nutrition requires a political environment grounded in multi-stakeholder platforms. The dialogue around hunger and nutrition should be open, for different groups to share the responsibility of scaling up nutrition throughout the entire world.
  3. Alignment of Actions Across Sectors and among Stakeholders. The country plans to improve nutrition should reflect frameworks of mutual responsibility and accountability among stakeholders.
  4. Increased Resources for Nutrition and Demonstration of Results. Multiple sectors and stakeholders should increase financial resources for the implementation of plans to improve nutrition.

Each participating country is required to meet state-specific goals and objectives for scaling up nutrition before they can partake in SUN Movement events, like the Annual Global Gathering. This event is where government leaders and multi-stakeholder groups meet to collaborate, share progress, learn from each other and offer new practices for improving nutrition.

SUN Movement has several mechanisms for maintaining oversight and staying on track to achieve its goals. The SUN Networks align resources and foster collaboration, the lead group provides strategic oversight and enforces accountability and the executive committee represents SUN Movement at the international political level. There is also a secretariat and multi-partnered trust fund.

SUN Movement acts like the United Nations for Hunger and Nutrition, with clear guiding principles to achieve goals through cooperation and collaboration. The multifaceted structure of SUN Movement accurately confronts the varied nature of hunger and malnutrition, making the organization an important player in the fight to improve nutrition worldwide.

Countries around the world are joining efforts in a program called Scaling Up Nutrition to improve the way malnutrition is being treated. By using nutrition-specific interventions and nutrition-sensitive approaches, Scaling Up Nutrition is on the path to decreasing nutrition problems that have horrible effects on societies.

The program consists of governments, civil society, the United Nations, donors, businesses and researchers. Together, the program can provide all the necessary resources to decrease malnutrition globally.

Scaling Up Nutrition was founded on the principle that all people have the right to nutritious food. The program has a focus on improving women’s and maternal health. Studies show that proper nutrition is essential during the 1,000 days from the start of pregnancy through the child’s second birthday. Poor nutrition during this time frame can lead to stunted growth and impaired cognitive development. Scaling Up Nutrition aims to prevent these from happening by expanding the knowledge and resources for women during and after pregnancy.

Their nutrition-specific interventions include support for exclusive breastfeeding up to 6 months of age and continued breastfeeding up to 2 years of age, fortification of foods, micro-nutrient supplementation and treatment for severe malnutrition.

Malnutrition is not caused solely by the lack of access to proper food. Recognizing this, the program is also incorporating nutrition-sensitive approaches. These include things like agriculture, empowering women, clean water and sanitation, education and employment, health care and support for resilience.

By combining all these facets that go hand-in-hand with malnutrition, Scaling Up Nutrition is able to work as a united front to put the proper policies forward, implement effective programs and provide necessary resources for the improvement of malnutrition.

Malnutrition is a core problem that can have severe consequences on individuals, families and entire societies. Poor nutrition often coincides with poverty. By improving nutrition around the world, Scaling Up Nutrition is taking a large step toward eradicating poverty around the world.

Of the deaths of children under 5 in Sierra Leone, 57 percent are the result of malnutrition, and both the ministry of health and government officials in Sierra Leone have begun work to reduce this horrifying statistic by joining Scaling Up Nutrition and by signing the Nutrition for Growth agreement.

As Sierra Leone recovers from its civil war, which ended in 2002, officials are attempting to shift the focus from malnutrition treatment to malnutrition prevention. Officials have been tracking the correlation between sanitation, education and malnutrition in order to improve prevention techniques.

In an interview with The Guardian, Aminata Shamit Koroma, the director of food and nutrition at the ministry of health in Sierra Leone, noted that women with a higher level of education were more likely to have access to adequate sanitation and less likely to have malnourished children.

In his efforts to prevent malnutrition in children, Koroma has been centering her campaign on breastfeeding and emphasizing to mothers the importance of breastfeeding their infants during the first six months of life. She has been spreading awareness through radio commercials and mother support groups.

Koroma has also been encouraging grandmothers to attend these mother-to-mother support groups so that they can impart their knowledge of child nutrition onto new mothers who might not be aware of the nutrients their children need. The Sierra Leone National Food and Security Food Policy of 2015-2016 also targets fathers so that they support their wives in breastfeeding. Besides emphasizing the future health of their children as a motivating factor, the initiative informs the families that if the mother is breastfeeding her child, they do not have to buy extra food for the child during the first few months of life.

The nutrition policy will also regulate the marketing of supposedly comparable and superior breast milk substitutes in order to ensure that mothers are not tricked by false sales promises. While Koroma knows it is unlikely that infant malnutrition will be eradicated within the next year or two, she recognizes the importance of the steps she is taking as she encourages the people of Sierra Leone to begin to change how they view infant health.

Malnutrition in Zambia runs rampant among children younger than 5 years old. Stunted growth as a result of chronic malnutrition affects one million of those young children. Chronic malnutrition is the result of a combination of insufficient food, poor health and inadequate health practices. Projects and initiatives have been started and continue to address the problem of malnutrition in Zambia.

Scaling Up Nutrition (SUN)

Through the National Food and Nutrition Commission (NFNC), the SUN initiative has been able to implement policies aimed at strengthening agricultural development in Zambia as well as increasing food insecurity. The SUN initiative focuses on the first 1,000 days of a child’s life from the time of conception, a period of time that is critical to human development.

Realigning Agriculture to Improve Nutrition (RAIN)

The RAIN project aims to reduce stunting in children through integrated agriculture, health and nutrition initiatives during the period from conception until 24 months of age. The project is a partnership between Concern Worldwide, which implemented the project, and the International Food Policy Research Institute. The Institute leads the monitoring, education and evaluation components during the five-year span of the project, from 2011 until 2015. The project is located in the Mumbwa district in the Central Province of Zambia, where the prevalence of malnutrition is 59 percent of children who are less than 5 years old.

Purchase for Progress (P4P)

A World Food Programme (WFP) global pilot project called Purchase for Progress helps smallholder farmers improve their opportunities and access to lucrative markets. WFP supports the Zambian government in assisting poor households to overcome malnutrition. The P4P project brings food supplies from smallholder farmers to school nutrition programs. Other activities through the project include the empowerment of women through animal traction, increasing market access and promoting sustainable agriculture methods.

Mobile Delivery and Trucking (MDT)

The MDT project is unique to Zambia and it has transitioned from an electronic food voucher distributor to a service that provides technical support to national agriculture, education, health and social programs. The program engages the private sector for the distribution of affordable, acceptable and accessible nutritious food products.

Those are just a few of the organizations that work on combating malnutrition in Zambia. Poor nutrition in children has been shown to affect mental health as well as physical health. With improvements in nutrition come more opportunities for children to advance in school and eventually be the intellectual force that drives Zambia’s economy.
