However, the population suffers from stunted growth and undernourishment because of the quality of their diet. This leads to a condition called hidden hunger, in Georgia.

Background

Hidden hunger in Georgia results from a lack of essential vitamins and minerals in its accessible food. The people there often do not consume enough protein, iron and vitamin A. This can cause tangible issues. For example, half a million Georgians are malnourished and infant mortality is twice the EU average. Additionally, a significant number of children under five years old are anemic.

Most of the foods that Georgians eat are quite high in starch and have little nutritional value. The two most popular dishes in rural Georgia are fried potatoes and lobio, which is made of boiled beans. Overreliance on these types of foods have made cardiovascular disease the most common chronic disease in the country. Currently, it accounts for 69% of Georgia’s mortality.

The main cause of the dietary insufficiencies in Georgia is a lack of access to meat and meat-based products. Unfortunately, these products are rather expensive at local markets. With the average household income being just $6 per day (⅓ of the population earns only $2.5 per day), the consumption of meat is rather impractical for most people.

Furthermore, the gross domestic product of Georgia was just $16.21 billion in 2018, with a per capita GDP of $4,723. For comparison, the 2018 GDP per capita for the European Union was $35,616.

Although the country’s GDP is growing overall, economic downturns, such as the 2008 Russo-Georgian War, the 2015 stagnation and the 2020 pandemic, reduce the value of the Georgian Lari. These kinds of shifts can create vulnerable conditions for Georgia’s population and reduce food security.

Solutions

Fortunately, governmental and nonprofit organizations across the world are taking steps to improve the dietary standards and hunger in Georgia. Action Against Hunger has had a Food Security Program in the country since 1994, established shortly after the dissolution of the USSR and the collapse of collective farming in the region. It was able to help 5,937 people in 2018.

BRIDGE is a Georgia-based NGO that publishes comprehensive studies detailing the dietary habits of Georgians. It also publishes policy recommendations, which range from developing monitoring systems for the Georgian diet to embedding nutrition into the Ministry of Education’s agenda.

The Georgian Agricultural and Rural Development Alliance (GAARD), of which BRIDGE is a member, was able to register a “Food Security Bill” in Parliament in 2017. This bill aims to reduce Georgia’s reliance on imported food and improve the country’s nutrition self-sustainably.

The Impact of COVID-19

Although the country has only 879 confirmed cases of COVID-19 and 14 deaths as of June 16 2020, the global pandemic may put national food security at risk if another wave of the virus hits the region.

There are some subsistence farmers in the country, but many people buy their food from street markets or bazaars. Places like these are potential hotspots where the virus can spread. However, it is essential that these markets remain open because if they were shut down by a government mandate, many people would struggle to achieve their daily food quantity as well as combat hidden hunger in Georgia.


Hidden hunger presents itself in Georgia due to a lack of essential minerals and vitamins in its available food. Cardiovascular disease accounts for 69% of Georgia’s mortality. COVID-19 has the potential to increase the impact of hidden hunger if markets are shut down. While Georgia is facing a struggle with hidden hunger, organizations like Action Against Hunger, BRIDGE and GAARD are working to improve the quality of food in the country in order to make a positive impact.

With one of the highest concentrations of millionaires in the world and a reputation for being a “Food Paradise,’” it is difficult to imagine that food security is an issue in Singapore. However, hunger persists despite Singapore’s reputation as an affluent and food-secure nation. This hidden hunger in Singapore is a result of food insecurity and has caused malnutrition throughout the country.

Hidden Hunger in Singapore

Singapore is ranked as the world’s most food-secure nation, yet many Singaporeans still struggle to access a sufficient and nutritious diet. This “hidden hunger,” or the high rate of malnutrition, has created a significant issue for the nation. According to the U.N., about 4.1% of Singaporeans experienced moderate to severe food insecurity between 2016 and 2018. Food security is more than having access to the amount of food needed to survive; it is having nutritionally adequate food that is vital for a person’s growth and development.

A large part of Singapore’s population experiences food insecurity first-hand. Researchers from the Lien Center for Social Innovation reported that only 2.5% of the survey respondents from four low-income neighborhoods had no food insecurity, while 80% of respondents experienced mild to moderate food insecurity. The researchers found that within the last 12 months, one in five low-income households in Singapore had to go a whole day without eating or could not eat when hungry due to a lack of resources.

However, food insecurity is not limited to low-income households. In fact, approximately 27% of the study participants had an average monthly income of $2,000 and above. This suggests that financial constraints are not the sole cause of food insecurity in Singapore.

Food Insecurity Leads to Malnutrition

This widespread hunger in Singapore leads to a high rate of malnutrition, especially in children and the elderly population. ONE (SINGAPORE) reported that one in 10 Singaporeans lack sufficient access to essentials, including healthy and nutritious food. This makes access to healthy food an unattainable reality for many.

Malnutrition as a consequence of an unhealthy or insufficient diet creates even more health-related issues for at-risk populations. ONE (SINGAPORE)’s website reports that upwards of 23,000 children in Singapore are malnourished as a result of food insecurity. This is a staggering number for such an affluent country. Around one in three elderly Singaporeans are at risk of being malnourished. In 2015, about half of the elderly population admitted to hospitals “were eating poorly,” making them more vulnerable to medical complications and other adverse outcomes.

Food Support Systems: Lacking Coordination

Despite the abundance and diversity of food assistance groups in Singapore, including nonprofit organizations, charities, soup kitchens, Meals-on-Wheels providers and informal volunteer groups, many people experiencing food insecurity remain hungry. According to the Lien Center for Social Innovation, more than half of the survey participants who experienced severe food insecurity received infrequent or no support at all.

In spite of the support systems in place (approximately 125 in 2018), the results of this report suggest they may be inefficient in addressing Singapore’s hidden hunger. Some attribute the inefficiency to the lack of coordination between systems. Many of these food support groups operated independently and there was no information-sharing network in place. This often created more problems: duplication of assistance, food waste and in some cases, little to no aid. In order to better coordinate efforts, stronger communication between different food aid organizations is needed.

Finding Common Ground

In 2018, officers from the Ministry of Social and Family Development started engaging several food aid organizations informally. This created the foundation for a multi-agency workgroup in 2019 which brings together food support organizations and agencies. The purpose of this workgroup is to provide a platform for collaboration to end food insecurity and food waste in Singapore.

While the workgroup is still in its infancy, it has made headway in coordinating efforts among the groups. The stakeholders have worked together to address food waste by compiling a list of sources that are willing to contribute unwanted food. In addition, they are working to map food groups and their needs in order to eliminate duplication of assistance and sourcing issues. These efforts make Singapore’s food assistance programs more efficient and effective.

 

Many helping hands devoted to alleviating hidden hunger in Singapore. However, the lack of coordination among these well-intentioned groups sometimes leads to mismatches between the providers and the beneficiaries. By recognizing the “hidden hunger” in Singapore and coordinating governmental efforts, the nation and its charities may be able to more efficiently address food insecurity in the nation.

Project Healthy Children

Global hunger is one of the most pressing and visible poverty-related issues in our world today. People can easily recognize the defined ribs, sunken eyes and bone-thin limbs of starvation. However, there is another side to hunger that is not as obvious: micronutrient deficiency.

Micronutrients are vitamins and minerals such as zinc, iron, iodine, vitamin A and folic acid. In developed nations like the United States, most people get these critical nutrients from maintaining a well-rounded diet or taking a daily supplement. But it isn’t always that simple in some other parts of the world. In fact, micronutrient deficiency remains a big problem in Eastern and Southern Africa but often does not get the attention it deserves because the effects are not immediately visible. For this reason, micronutrient deficiency has been nicknamed “hidden hunger.”

Hidden hunger has real and long-lasting consequences. Insufficient amounts of vitamins and minerals can result in learning disabilities, mental retardation, low work capacity, blindness and premature birth. These deficiencies lower overall health and weaken the immune system, thus making it much harder to survive infections like HIV and measles. They can cause extreme birth defects in children and are the leading cause of maternal death during childbirth.

Background

Clearly, micronutrient deficiency is a pressing issue that deserves the attention necessary to mitigate it. An organization called Sanku’s Project Healthy Children (PHC) is doing just that through a process known as food fortification: essentially, they add critical micronutrients to the flour people already consume.

PHC is based in Tanzania and currently supplies almost 2 million people with fortified flour to help them get the vitamins and minerals they need. Flour is a staple food that many people consume regularly; according to the PHC website, “over 50 million Tanzanians eat maize flour every day,” but more than 95 percent of it is produced without added nutrients in small, rural mills. Countries like Tanzania are in desperate need of better access to micronutrients—here, about 35 percent of children under 5 years old have stunted growth due to under-nutrition. Project Healthy Children uses the mills and distribution systems already in place to simply add essential micronutrients to the flour with no additional cost for the consumer. This way, people can get the nutrition they need without changing their eating or purchasing habits.

Why Food Fortification?

  1.  It is cheap: Food fortification is very inexpensive, typically costing no more than $0.25 per person annually. In other words, one quarter donated is enough to supply someone with adequate nutrients for an entire year.
  2. It is effective: Improving nutrition can be highly beneficial to overall health, work capacity and productivity. Women who sustain good nutrition before getting pregnant greatly reduce the risk of maternal death and birth defects.
  3. It has a huge payback: The economic rewards of food fortification are astounding. The WHO estimates that the consequences of micronutrient deficiency (birth defects, learning disabilities, premature death, etc.) can cost a country about 5 percent of its GDP per year. Supplying people with critical vitamins and minerals puts less pressure on a country’s health care system and allows for a more productive workforce. In addition, the Copenhagen Consensus estimated that for every dollar spent on nutrition in young children, a country will save an average of $45 and sometimes as much as $166.

The Future of Project Healthy Children

In the past few years, Project Healthy Children has become even more streamlined in its approach to food fortification. A partnership with Vodafone, a mobile network based in the United Kingdom, allows PHC staff to remotely monitor flour mills so that they instantly know when a machine is down or a mill is low on nutrients. The partnership saves money, time and manpower, allowing PHC to run more smoothly.

Project Healthy Children currently helps nourish about 1.7 million people in sub-Saharan Africa but hopes to reach 100 million people by 2025, an ambitious goal that would be instrumental in lifting communities in Southern and Eastern Africa out of extreme poverty.

  1. Upwards of 23,000 children in Singapore are malnourished. Additionally, basic needs are inaccessible to 1 in 10 Singaporeans, including essentials such as food and shelter. But, the issue is larger than just putting food on the table and providing for the family. Access to healthy and nutritious food is not a reality for most, and consequently, such unhealthy diets lead to further health issues and chronic disease.
  2. Hidden hunger is real. Hidden hunger is when someone is suffering from malnutrition without feeling hunger. This has recently become a critical problem, specifically among migrant workers where the common food staple is rice. A diet reliant solely on rice, however, lacks adequate and essential nutrients, which leads to malnutrition. To combat this threatening social issue, an organization called BoP HUB is teaming up with a Dutch-based life sciences company by the name of DSM. The two organizations are focusing on fortifying rice, the Singaporean staple, so workers and those suffering from hidden hunger will have access to a nutritious alternative to regular rice, essentially turning meals from empty carbs to healthy carbs.

Relative poverty, more apparent in developed nations, regions and cities, is essentially the cutoff line for how much a household should be able to afford in terms of basic necessities. Associate Professor John Donaldson of the Singapore Management University (SMU) School of Social Sciences states that “Singapore’s economy developed rapidly, and the ‘Third World’ form of poverty has disappeared. Yet, many people fall into a type of ‘First World’ poverty.” According to economists and statistics, between 10-14 percent of Singaporeans suffer from severe financial trouble and pressure and are often unable to meet basic needs. Ultimately, hunger remains one of the most prominent issues.
  5. The Food Bank of Singapore has accepted the challenge of eradicating hunger. This organization receives donations of surplus food from retailers, distributors and manufacturers. Despite losing commercial value, the foods are still safe to consume. Through the diversification of food sources, the economy and food security are not highly impacted by other nation’s economic decline because the variety of import countries allows for flexibility.
  7. Local food production has increased. The Agri-Food and Veterinary (AVA) and the Food Fund have allowed Singapore to increase its local vegetable production by 30 percent over the past decade. Upwards of 40 percent of local farms are benefiting from the help of investment in advancements and new farming techniques such as hydroponics.
  8. Rising obesity in children and young adults foretells an increase in diabetes. Rates for diabetes in adults have already risen from 8.6 percent in 1992 to 12.9 percent in 2015. With one in three affected by malnutrition globally, hidden hunger is a prevalent but treatable epidemic. Africa is currently home to the highest levels of hidden hunger.

The UN stated in a report that, “of the 34 countries that account for 90% of the global burden of malnutrition, 22 are in Africa,” with 56 million children suffering stunted growth as a result.

But the good news is that an affordable solution appears to be on the horizon.

Marie Konaté, founder of Protein Kissèe-La (PKL), has created a key vitamin and mineral fortified cereal for children. Sourced locally from Côte d’Ivoire, the children’s cereal is targeted at the group most susceptible to hidden hunger: children under the age of 5.

As 46% of the African staple-based diet is composed of cereals, according to the Food and Agriculture Organization (FAO), Marie’s cereal is an affordable and delicious way to fight micro-malnutrition.

The Global Alliance for Improved Nutrition (GAIN) has teamed up with PKL to provide the powdered supplements that are added to the cereal, thus keeping the costs even lower to consumers.

The fortification of food staples has already been implemented in a variety of products, from Nestlé’s bouillon cubes with added iron and iodine in Western and Central Africa to cooking oil with iron and vitamin A in Senegal.

Yet, one of the simplest ingredients used globally could be one of the most effective in fighting hidden hunger: iodized salt.

Commonly used in developed nations, iodized salt looks, smells and tastes the same as non-iodized, costing only 5 to 10 cents per person per year, reports Aljazeera America.

Iodine Deficiency Disorders (IDDs), including goiter, hypothyroidism, reproductive problems in adults and mental and structural defects in infants, can all be combated through the simple consumption of iodized salt.

GAIN estimates that the number of children dying under the age of 5 could be halved if they received better nutrition.

Given that a whole host of staple ingredients, including salt, grain and oil are able to be fortified without compromising the taste and appearance of the product, it’s only a matter of funding to drastically reduce the numbers of micronutrient deficient people globally.

At the 2015 Future Fortified summit in Tanzania, GAIN, the African Union, the Bill and Melinda Gates Foundation, the World Health Organization, UNICEF, the World Food Program and USAID hashed out a viable global plan to put an end to hidden hunger.

They estimated that for every dollar spent on ending malnutrition, $138 would be saved on healthcare and previously lost productivity.

Since then, the African Union has requested $150 million in donations to fund food fortification and progress analytics in 25 nations.

Going forward, as more countries commit to mandating fortification, hidden hunger is closer than ever to being a problem of the past.
