HPV Vaccines in Laos

Despite the changing direction of attention away from other illnesses and towards COVID since the beginning of 2020, Laos has made significant forward strides regarding HPV (human papillomavirus) and Laotian health. In March 2020, the program for HPV vaccines in Laos was founded as an active response to the high number of deaths due to cervical cancer. In Laos, at least 320 women are infected with HPV every year; 182 or around half of the sufferers die from the illness. The vaccines were first distributed in a high school in the Xaythany District in Vientiane Capital. Schools across the country administered the vaccinations shortly afterward to increase HPV prevention in 360,000 girls from the ages of 10 to 14.

Why Does This Matter?

The new vaccination program is significant considering that cervical cancer is the third most common cancer in Laos. It is also second-highest cancer to cause premature death or disability next to liver cancer. Furthermore, Laos is the eighth Southeast Asian country to have high rates of cervical cancer according to an age-standardized graph. More middle-aged women from the ages of 40 to 64 years old are diagnosed with cervical cancer each year: 197 a year compared to 62 older women and 61 younger women. Of the 197 middle-aged women who contract cervical cancer, 101 of those women die from the disease.

Logistics of the New Vaccines

Gavi, the Vaccine Alliance, and the government both contributed to the price of the HPV vaccines in Laos. UNICEF helped the Ministry of Health as well as the Ministry of Education and Sports to implement the vaccination of adolescent girls. The government vaccinates girls early on to battle HPV as a preventative measure as opposed to treating it at advanced stages later on in life. The vaccines work to prevent the infection of two strains of HPV, 16 and 18, which cause 70% of all cervical cancer cases due to HPV.

Vientiane Vaccinations

This is not the first time that there have been HPV vaccines in Laos. They were first introduced in 2013 as part of a demonstration of the vaccine’s effectiveness. The vaccines were confined within the Vientiane province until 2015 when the government completed the demonstration successfully. This most recent round of vaccinations was the first time the HPV vaccine was produced nationally.

Increasing Rate of HPV Vaccination

HPV vaccination in Southeast Asia is relatively new. Most countries are giving priority vaccination to illnesses like DTP (diphtheria, pertussis, and tetanus) and measles. These illnesses are also the three garnering the most vaccinations worldwide. Because HPV is a newer vaccine for low and middle-income countries, there is still much to be done about the vaccination process. For example, low and middle-income countries still experience a dropout in between the first and final doses of HPV. This leaves many women at risk. Still, due to GAVI’s implementation and increasingly lower prices, countries like Laos can expect a decrease in cervical cancer fatalities.

HPV is a serious, cancer-causing infection afflicting countries all over the world. Fortunately, Laos is keeping up with other low and middle-income countries with their vaccination program. With the help of GAVI and UNICEF, Laos can increase vaccine availability and effectively combat one of the deadliest cancers in the country.

Vaccine Hesitancy in Developing CountriesVaccine hesitancy, the reluctance or refusal to vaccinate despite the availability of vaccines, has been around since the invention of the vaccine. Recently, there has been an increase in vaccine hesitancy in developing countries due to safety concerns and long-term effects. With this skepticism, outbreaks of vaccine-preventable diseases that were once thought mostly eradicated such as measles, pertussis and diphtheria, have increased in frequency around the world.

This year, the World Health Organization named vaccine hesitancy one of the top 10 threats to global health. Vaccines remain the safest and most cost-effective ways of preventing diseases. Currently, vaccines save between two and three million lives a year.

Furthermore, if coverage improves, vaccines can save an additional 1.5 million lives per year. The reasons for vaccine hesitancy are complex, but a vaccine advisory group identified complacency, inconvenience in accessing vaccines and a lack of confidence as some of the root causes. With these factors identified, if now becomes a question of how to fight these causes of vaccine hesitancy in developing countries.

Complacency

Some health experts have theorized that vaccine skepticism stems from the fact that vaccines are so effective that parents no longer remember or fear vaccine-preventable diseases like measles and polio. Instead, they may focus their anxieties on the safety of vaccines, effects or the number and timing of injections.

Without a proper understanding of the devastating effects of these diseases, parents have less of a motivation to vaccinate. EU Health Commissioner Vytenis Andriukatis said at an international health conference, “We have become victims of our own success.”

Convenience

Vaccine hesitancy in developing countries is often bolstered by the need for medical infrastructure. Inadequacy and the inequities in health systems like poverty, the disparity in infant mortality and life expectancy and a need for trained providers damages community trust.

To combat this, many countries have implemented mandatory vaccinations in schools, with mixed results. In India, for example, during their measles and rubella vaccination campaign, parents objected to their children being vaccinated without their consent, stalling the campaign in court.

Rwanda, on the other hand, was successful in its HPV vaccination campaign by coupling it with an information campaign. The campaign targeted parents and explained the need for this vaccine.

Confidence

Much of the anti-vaxxer movement is built in misinformation. Vaccine hesitancy in developing countries often stems from rumors about vaccines. For instance, that the polio and HPV vaccines caused infertility and impotency. The movement stems from general concerns about the safety of vaccines.

Health workers, especially those operating in the communities, are the parents’ most trusted health advisors and act as the biggest influencer of vaccination decisions. Because of their position, they must be supported so that they may provide parents with credible information. The most effective campaigns to fight vaccine hesitancy in developing countries are the ones that tailor to the community. Healthcare workers in the community are vital for implementing these campaigns.

In Rwanda, for example, healthcare workers went door to door to explain the benefits of the HPV vaccine. They focused on the fact that the vaccination is meant to prevent cancer. The workers brought diagrams of the female reproductive system. They also dispelled rumors by explaining that cervical cancer is far more likely to cause infertility than vaccines. Because the campaigned was tailored to the community, it was met with a lot of success.

Overcoming Vaccine Hesitancy

While there is an increase in vaccine hesitancy in developing countries and around the world, immunization campaigns are experiencing successes. Transmission of wild poliovirus, for example, may stop in Afghanistan and Pakistan this year. Additionally, cervical cancer may be eliminated in 181 countries by the end of the century.

To keep this up, health officials have to be proactive in fighting vaccine hesitancy in developing countries by fighting complacency, making access to healthcare more convenient and building confidence by creating programs tailored to the community.

All You Need to Know About HPV in the Developing World
Human papillomaviruses (HPV) are DNA viruses that infect skin or mucosal cells. Depending on the severity of the infection, HPV can lead to either cervical cancer and other head and neck cancers or low-grade cervical tissue changes and genital warts. Virtually all cervical cancer cases result from a sexually transmitted infection with HPV.

Cervical Cancer and HPV in the Developing World

Globally, cervical cancer is known as the second most common cancer among women, with about 500,000 new cases being diagnosed annually. Of the total deaths that occur due to cervical cancer each year, more than 80 percent are concentrated in developing countries.

Immunization coupled with regular screenings and consistent treatments are the best strategies for reducing the burden of cervical cancer and HPV in the developing world. In resource-poor countries that lack adequate access to cancer screenings and treatment services, it is even more essential that younger girls be immunized before they are sexually active and are exposed to HPV.

The HPV Vaccine

The HPV vaccine protects against the strains that cause up to 90 percent of cervical cancer cases. It is typically available in most routine immunization programs of high-income countries. Historically, the major barriers to reducing the burden of cervical cancer and HPV in the developing world are due to the high costs of the HPV vaccines and the difficulty of reaching adolescent girls.

The GAVI Alliance–formally known as the Global Alliance for Vaccines and Immunization–is a partnership of national governments, the World Health Organization (WHO), the World Bank Group, the Bill and Melinda Gates Foundation, the vaccine industry and many public health institutions. GAVI provides technical and financial support for vaccines in countries that have a gross national income of less than $1,000 per capita and other poverty-stricken countries including China, India and Indonesia.

Thanks to the efforts of GAVI, the HPV vaccine is at a record low price and the poorest countries are able to access it for as little as $4.50 per dose. Additionally, the WHO decided to change the recommended dosage of the HPV vaccine from three to two doses, which helped facilitate the country rollout of the vaccine as well as significantly reducing costs.

The first HPV vaccine demonstration program took place in Kenya in 2013, and since then, 1,000,000 girls have been vaccinated. By the end of 2016, GAVI had initiated HPV vaccine demonstration programs in 23 countries, which is the first step toward introducing the vaccine to national immunization programs. So far, Honduras, Rwanda and Uganda have introduced the HPV vaccine into their national immunization programs.

Potential Roadblocks in the Push for the HPV Vaccine

Unfortunately, the transition from the demonstration programs to national introductions is taking longer than expected for some countries. Consequently, GAVI has developed a new approach to HPV vaccine support, which draws from the valuable lessons learned from previous demonstration programs.

Some of these lessons include:

  1. The fact that school-based delivery works very well when administering the vaccine to young girls. It is more cost effective to integrate HPV immunization efforts into routine immunizations at existing health clinics and schools.
  2. When promoting HPV vaccination programs and cervical cancer prevention, the facilitation of effective and factual communication within the community is particularly critical.
  3. GAVI has made tremendous progress in reducing the prevalence of HPV in the developing world through its vaccination initiatives. Eight GAVI-supported countries have integrated the HPV vaccine into their national vaccination programs and 30 countries have started a demonstration program.

However, despite the strong signs of interest from GAVI-eligible countries and the rapid and effective integration of the HPV vaccine, GAVI’s original goal of immunizing 40,000,000 girls by 2020 may be at risk due to supply constraints.

GAVI chief executive Dr. Seth Berkley stated, “Scaling up cervical cancer prevention and control strategies should not be delayed, as we have the tools to achieve this goal. With the right commitment from vaccine manufacturers as well as political support, strategic partnerships and investments, this particular battle to improve women’s health can be won.”

Thus far, GAVI has helped low-income countries access the HPV vaccine at affordable and sustainable prices. Dr. Berkley is confident that the organization is capable of meeting its goal. GAVI is dedicated to ensuring that its progress is maintained and that millions of girls in the poorest of countries are protected from the perils of HPV and cervical cancer.
